PoNS Patient Therapy Access Program

The PoNS Patient Therapy Access Program (PTAP) has been established for a limited time to benefit those experiencing gait deficit and/or impairment due to mild to moderate symptoms of multiple sclerosis who are potential first-time users of PoNS Therapy™ in the US. If you meet the eligibility criteria, you may have access to PoNS Therapy at a greatly reduced purchase price.

To proceed with your enrollment into the program, you will be required to:

1. Provide a valid prescription for the PoNS® device
2. Provide a Letter of Medical Necessity (LMN) including details of the diagnosis, medical history, current clinical presentation, and requirement for PoNS Therapy
3. Sign a consent form/HIPAA release to agree and allow Helius’ access to your medical records for purpose of assessing your eligibility, therapeutic benefit, and safe exposure to therapy for a period of two years

If you meet the criteria to take advantage of our PTAP program, you will need to provide a signed statement in which you agree not to seek or obtain reimbursement for the cost of the device or any components thereof from any federal or governmental healthcare program, any private or commercial insurance or any other party.
